Verdict
TUDCA wins for direct cholestatic liver injury, rapid reduction in bile acid toxicity, ER stress rescue during harsh methylated cycles, and acute gallbladder protection. Its clinical mechanism is uniquely suited for oral steroid or prohormone hepatotoxicity—with almost immediate results on GGT/ALT when injury is present. Side effect profile is excellent; only notable issue is GI tolerance at very high doses.
N-Acetylcysteine wins for ongoing oxidative stress reduction, glutathione repletion, and multi-system protection.
